Basically you need a lot of tokens to sacrifice and at least a little bit of control or health gain in order to survive long enough to see your Altar work properly; also you need different magicka cost creatures so you won't have any useless turn.
It basically works this way: sacrifice > summons > triggers the summoning effect. It does great in combo with cards like Cauldron Keeper and anything that has a summon ability (Odahviing, Ancano and Divayth Fyr the best examples).
Absolutely amazing card. You have to build your entire deck around it, so Last Gasp Assassin or Sorcerer is the best deal. Scouts can make good use of it too. The trick is get 2 trios of creatures on each cost with good summon and/or last gasp abilities. Combo that with Dark Rebirth, Cruel Firebloom, Necrom Mastermind and Journey to Sovngarde and voil√°.
Personally I love to summon a free Indoril Archmage over a Brutal Ashlander.
